Testosterone propionate was first described in 1935 in a series of experiments to increase the therapeutic effects of testosterone by slowing its release into the bloodstream. Two years later, Schering AG of Germany launched the first testosterone propionate product under the name Testoviron. Propionate was also the first commercially available injectable ester of testosterone on the U.S. prescription drug market and remained the predominant form of testosterone worldwide until 1960. For example, in the early 1950's, when steroids were first used by Americans on athletes, readily available anabolic/androgenic steroids were methyltestosterone, testosterone propionate, and testosterone suspension. Interestingly, testosterone propionate was also available orally during this time, but disappeared from the U.S. market in the 1980s.

Early prescribing guidelines for testosterone propionate indicated some therapeutic uses. It is mainly suitable for cases of male androgen deficiency, such as decreased libido and impotence in adults, and cryptorchidism in adolescents. But there are also uses for the treatment of menopause, menorrhagia, chronic cystic mastitis (fibrocystic mastitis), endometriosis and hyperlactation, covering a large number of cases where the male hormone testosterone is applied to female patients. Over the years, the FDA narrowed its use. By the 1980s, testosterone propionate was used only in male patients.

However, it must be emphasized that its influence in the market is mainly due to its history rather than unique performance. Testosterone propionate was the first accepted ester of testosterone and thus has decades of history as a therapeutic agent.
